Ashwagandha KSM-66 for Cortisol and Stress
What the randomised trials show about KSM-66 ashwagandha root extract for perceived stress, serum cortisol and sleep quality, plus sensible dosing and safety.
In several randomised controlled trials, a standardised ashwagandha KSM-66 root extract (typically 300-600 mg a day) lowered participants' perceived stress and reduced serum cortisol, the body's main stress hormone. Trials also report better sleep quality and calmer self-rated mood over 6-8 weeks. The effect is modest but consistent, and the extract is generally well tolerated in healthy, stressed adults.
What is ashwagandha KSM-66?
Ashwagandha (Withania somnifera) is a herb used in Ayurvedic tradition and now one of the most-studied "adaptogens" - plants thought to help the body cope with stress. KSM-66 is a specific, branded full-spectrum extract made from the root only and standardised to a set level of withanolides (its active compounds).
So what? The brand matters because most modern stress and cortisol trials used either KSM-66 (root) or a comparable standardised root extract. When you read "ashwagandha lowers cortisol," the evidence behind that line is usually a standardised root extract at a known dose - not loose powder of unknown strength.
Does ashwagandha lower cortisol?
This is where the evidence is strongest. In a 60-day randomised, double-blind, placebo-controlled study of 64 chronically stressed adults, a high-concentration full-spectrum ashwagandha root extract (300 mg twice daily) cut serum cortisol by about 27.9% versus placebo, alongside large drops on stress-rating scales.
A later trial in healthy, stressed adults found serum cortisol fell with both 250 mg/day and 600 mg/day, with the 600 mg dose showing the clearer reduction. A separate study of a standardised extract also reported greater falls in morning cortisol versus placebo.
So what? Across different teams and products, the direction is the same: a standardised root extract tends to nudge cortisol down in people under stress. That is an association measured in blood, not a promise about how you'll feel - but it is a real, repeated finding.
Can it actually reduce how stressed I feel?
Yes - the subjective scores tend to move too, not just the bloodwork. In the 60-day trial above, the ashwagandha group reported roughly a 44% drop on the Perceived Stress Scale, compared with about 5.5% on placebo. Other randomised trials using validated anxiety and stress questionnaires (such as the Hamilton Anxiety scale and DASS-21) reported greater improvements with ashwagandha than placebo.
So what? Lower cortisol numbers would mean little if people didn't feel any different. The fact that both the hormone and the self-rated stress tend to improve together makes the signal more believable. It's also worth saying plainly: this is about perceived everyday stress, not a treatment for any diagnosed condition.
What about sleep?
A 2021 systematic review and meta-analysis pooled five randomised controlled trials (400 participants) and found ashwagandha extract produced a small but statistically significant improvement in overall sleep (standardised mean difference -0.59). Four of those trials used KSM-66 root extract.
The benefit was clearer in certain groups: people taking 600 mg/day or more, those treated for eight weeks or longer, and adults with diagnosed insomnia. The review also noted better mental alertness on waking.

How much ashwagandha should you take?
Most positive trials used a standardised root extract at 300-600 mg per day, taken either once daily or split into two doses, for at least 6-8 weeks. Some studies dosed in the evening; others split morning and evening.
- Form: look for a standardised root extract (KSM-66 is the most-studied) rather than unstandardised powder, so you know the withanolide content.
- Dose: 300-600 mg/day reflects the trial range; higher isn't automatically better.
- Time: give it 6-8 weeks before judging, since benefits built up over that window in the studies.

Is ashwagandha safe?
In the randomised trials, ashwagandha was generally well tolerated, with mild side effects (such as mild gut upset, headache or drowsiness) that were similar to placebo. That's reassuring for short-to-medium-term use in healthy adults.
A few sensible cautions still apply. There are rare published case reports of liver injury linked to ashwagandha-containing products, usually reversible after stopping; the LiverTox database rates it a probable, uncommon cause of clinically apparent liver injury. Ashwagandha may also influence thyroid hormone levels, so it warrants care if you have a thyroid condition or take thyroid medication. It is best avoided in pregnancy.
So what? The overall safety record in trials is good, but "natural" still means "active." If you take regular medication, have a liver or thyroid condition, or are pregnant or breastfeeding, check with your GP or pharmacist first.
